We are now linked to MyHealthOnline. This service supplies the feature of requesting your repeat prescriptions online and also provides the following;

• Cancelling appointments
• Updating personal information

Once you have completed the form opposite please email it back to us or post it to us with a form of identification such as a driving license or passport. Or alternatively if you are unable to complete the form to send to us, send an email to the address provided above with your full name, date of birth requesting a My Health Online Pin to be set up for you. Please attach a form of identification to the email.
